The article examines promising research areas in the development of municipal management of the network of rural healthcare institutions in a region. It is substantiated that the key problem of rural healthcare is not only the insufficient number of medical organizations, but also the mismatch between the formally existing network and the actual accessibility of medical care for the population. Based on the analysis of statistical data, the study identifies limitations related to the territorial differentiation of the outpatient network, instability in the commissioning of new healthcare capacities, reduction of primary healthcare infrastructure, and declining staffing sustainability of mid-level medical personnel. The article shows that municipal management should be considered as a mechanism for spatial balancing of the rural healthcare network and for coordinating stationary, mobile, outreach, and digital forms of medical services. The conclusion is made that it is necessary to move from quantitative administration of institutions to management of actual accessibility of medical care, taking into account settlement patterns, transport connectivity, demographic burden, and social risks of rural areas
